The questionnaire is a vital part of your "blind inquiry" package.
The HAVEMANN Family Information Form I am researching the Havemann family, which came from Germany before the turn of the century. I would very much appreciate as much information as you can provide. Returning this form is, of course, not mandatory -- but it would certainly help me in my research, and I would very much appreciate it. Please return to: Paul Havemann, [FULL ADDRESS & ZIP]. A self- addressed, stamped envelope is enclosed. Your Name ___________________________________________________________ and Address ___________________________________________________________ Your Wife's Name (if married) _______________________________________ What is Your Father's Full Name? _________________________________________________________ Where & When Was He Born? ________________________________________ Whom Did He Marry (her maiden name, please)? _____________________ What is Your Grandfather's Full Name? _________________________________________________________ Where & When Was He Born? ________________________________________ Whom Did He Marry (her maiden name, please)? _____________________ What is Your Great-Grandfather's Full Name? _________________________________________________________ Where & When Was He Born? ________________________________________ Whom Did He Marry (her maiden name, please)? _____________________ As you can see, there are two purposes of this form: first, to determine where our families diverged. My own family, which is descended from Alfred William Havemann (born in 1906 in NY), lived in the Bronx before moving to northern New Jersey. Other Havemanns headed West -- I have found families in Michigan, Iowa, Ohio, California, Texas, and other states. The other purpose is simply to locate the various Havemann families around the country. It may interest you to know that the original branch of our family is still going strong in West Germany! Since you have read this far, I guess you're still interested -- so I'll go out on a limb here: if you have any old documents, photos, etc from your distant past, I would very much appreciate copies (or the chance to make copies) of them. Not only would they help my research, but, as my letter says, it's my intention to write a Havemann Family History, and photos, wills, letters & such will make it much more interesting. I will gladly send a free copy to anyone who responds. Thank you!The following should be printed on the reverse side of this form:
Additional Notes & Comments If you have any other information you would like to share, such as more details on your father, grandfather, and other ancestors; interesting family events; the names of other family members whom I could contact; books & articles written by (or about) Havemanns; or anything else, please use the space below.
